Know what you are measuring. 87 measures use source definitions or source-based calculations; 62 are visibly marked local templates with related references. This library includes quality, safety, access, experience, performance, financial and sustainability measures. Coverage is broad, not an exhaustive register of every profession. No universal pass/fail targets are applied.

How to choose and use an indicator
- Choose the profession and measure type, then read the definition before collecting data.
- Use matching periods, populations and units. Keep source records for both the numerator and denominator.
- A zero denominator is not available, not 0% compliance. A percentage above 100 may be valid for some ratios but invalid for a subset proportion.
- Pool matching numerators and denominators before calculating a multi-period rate; do not simply average monthly percentages.
- Use outcome, process and balancing measures together. A metric alone does not establish a cause or prove improvement.
- For local templates, agree the definition and target with the responsible service. Related references do not prescribe or endorse the exact template.
- Healthcare measures are aggregate measurement aids. Apply approved local policies, current surveillance definitions and risk adjustment where required.
- Some sources are historical or were accessible only as indexed definitions. The source register shows these limitations.
These calculators do not predict NHSN infections, assign regulatory compliance, diagnose patients or produce a combined quality score.
